1. Patient Portal
Give us your email and we will send on your cell phone or email, a Link to Patient Portal as soon as we make an appointment for you. Please go to this portal and fill out your demographic information (e.g., address, etc.) and Medical information (e.g., medications, Allergies, Past medical and surgical History, etc.)

If you need immediate medical attention, please go to the nearest Emergency Room or Urgent Care. We see patients by appointment only.

Directions to our office: We are located on 3r FLOOR in “MEDICAL ARTS COMPLEX” building next to (in the same campus and west of) “Tampa General” Hospital in Brooksville’ on Cortez Blvd (Hwy 50) at 17222 Hospital Blvd, Ste 322, Brooksville FL 344601.
Insurances we accept:
- We accept most insurance. However, please note that although we accept your ‘INSURANCE’ in general, now a days, Insurances have different ‘PLANS’. Please note that it is your responsibility to check YOUR SPECIFIC PLAN to ensure your benefits, network, deductibles, copay, coinsurance and pay your share accordingly during your visit. Please see your Health Plan’s Participating Provider List and call your insurance for accurate updated information.

If your Insurance is HMO, you will need AUTHORIZATION FROM YOUR PRIMARY CARE DOCTOR. Without this authorization, the insurance will deny payment and you will be stuck with the bill. To avoid this, please call your primary care doctor and ask them to fax us on 352-565-4449, following authorizations:
▫New patient visit code: 99205 x1 visit in next 1 month
▫Follow up visit code: 99215 x5 visits in next 12 months.
- This fax should have all your information and your insurance information.
- As soon as we receive this fax, we will call you to make your appointment within next 2 business days.
- You are responsible for payment of Copay and deductible if any at the time of your visit
- If you don’t have insurance, we can offer ‘discounted cash pay price’ or a workable payment plan

To make your visit efficient, effective and pleasant, Please bring with you following items (if applicable) for your appointment:
- Your Eye glasses and hearing aids if you need them
- Completed signed “New Patient Information form” on this page. Otherwise please come 15 minutes prior to appointment to fill the form in our office.
- Driver’s license
- ALL insurance & prescription cards
- Contact information (Name, address, phone, fax) of your PCP (primary care doctor) & specialist doctors you currently receive treatment from
- Referral letter from your PCP if you have HMO, as otherwise insurance will deny payment for you. IF you have PPO, You do not need this
- List of your medical problems with medications & dosage
- List of your medications and Allergies
- List of your past surgeries (if possible with date, location and surgeon’s contact details)
- XRays/MRI/CT scan/Labs if any done previously: Please bring Printed report on paper and images on a disc.
- Printed reports of previous treatments if any received previously for this issue, with provider’s contact details.

Pre-surgery Instructions:
- For 7 days prior to your surgery date Stop taking all blood thinners and anti-inflammatory medications that can compromise blood clotting with permission from your PCP or Medical Specialist (eg.: Cardiologist)
- Buy “HIBICLENS Antibacterial and Antimicrobial skin cleanser” (unless allergic) over the counter and Use it while showering to scrub well the operative area, armpits, groins, nostrils, all skin folds.
- see your PCP to get Pre-op testing AND CLEARANCE. If required, the hospital will call you within next few days to schedule your appointment for your ‘pre-op testing’ at ABOVE hospital. FOR ANY QUESTIONS ABOUT PRE-OP TESTING, CALL WANDA OR MICHAELA ON Tel: (352) 797-5601)
- Prior to your joint replacement surgery attend “JOINT CAMP” at above hospital 3rd floor at 1045AM on any WEDNESDAY. FOR ANY QUESTIONS, CALL MELISSA ON 352-544-6001, MFicarrotta@TGH.org
- For surgeries at Florida Endoscopy and Surgery Center, Register on website https://www.mychart.org/ and follow steps to enter your health history so that Anaesthesia can review it. This will avoid possibility of last minute cancellation of your surgery.
Post-surgery Instructions:
- Do not get your dressing wet at any time. While showering or bathing you may cover it with water proof plastic (You can buy this From any store or Amazon) to stop water entering the dressing.
- Do not soil or change dressing yourself or tamper with the dressing or wound. The DRESSING CAN BE CHANGED ONLY BY A PROFESSIONAL NURSE UNDER STERILE CONDITIONS. If dressing needs to be changed urgently because of soiling or oozing, please go to nearest Emergency or urgent care facility to get it changed.
- For pain relief after surgery you may buy Cold therapy pack from Amazon. It may be reimbursable through your insurance.
- After major surgeries such as but not limited to joint replacement, you need ‘home health care’ to change dressing (on Post op day #3,6,9, 12 or sooner if it is stained, and remove stitches/Staples on day#14) and start physical therapy immediately. The list of home health agencies is provided below. As we do not know which agency you would like and which takes your insurance, YOU NEED TO CALL THEM ASAP TO ARRANGE IT. Once you find / choose agency you like, please ask them to fax us on 352-565-4449 the order for Doctor’s signature. The Dr will fax it back to them as soon as we receive it.
